Ontario Student Transcript
Request Procedures
Ontario Student Records are kept by our board for 55 years after the student has left. Records for students who attended Lucas from June 2003 – present are kept on premise. Students who left or graduated before June 2003 would contact the TVDSB Record Centre at (519) 452-2000 extension 69688 to arrange to get their transcripts.
To obtain an Ontario Student Transcript from the Guidance Department you must visit our office, fill out a transcript request form, sign it, and pay $10.00 and it is done on the spot. It usually takes about 10 – 20 minutes. We are open from 8:00 – 4:00 Monday – Friday.
We will give you the transcripts or mail them to a post secondary institution on your behalf. Under no circumstances can we send a transcript by fax.
Parents can request a transcript for their student only if the student is under the age of 18. Due to Privacy Laws, we do not provide transcripts to third parties for students over the age of 18.
Adults over the age of 18 who need someone else to pick up their transcripts for them must authorize that on the transcript request for and sign that authorization. This can be done by email. To make arrangements for this call Ms. Charles at (519) 452-2000 ext. 60004.
Absolutely No transcripts will be processed until the $10.00 (for up to 3 copies) payment has been received. We take cash, cheque or money order. NO credit card or debit payments available.

Keeping Accurate Records
Article taken from Announcements
|
Please help us avoid attendance issues.
Lucas needs your help to prevent inaccurate TRUANCY information on your son or daughter’s attendance record.
- call the attendance line 519-452-2613
- please clearly state your name, your relationship to the student, the student’s name and the reason for the absence
- this should be done immediately, but definitely before 9 a.m. the following morning
- call in each day the student is away and/or with a list of the specific dates
Other ways to help?
- let us know if SynreVoice calls you with an erroneous report
- check the Parent Portal for attendance regularly and report any unexplained absences
Thanks for helping us help our students -- your children -- to be more successful!
